What You Need To Know
Once you have decided on the benefits you want to enroll in, gather the following information:
- Dependents: Full name, Social Security number (not required for newborns), birth date, and relationship to you.
- Beneficiaries: Full name, Social Security number, birth date, relationship to you, complete address and, if applicable, the name of the estate or trust.
How To Enroll
Whether you’re new to Republic Services, experiencing a life event change, or it’s Open Enrollment, you have three ways to enroll in, or make changes to, your benefits:
- Visit the Benefits Service Center website, log in, then follow the online enrollment steps. If you’re enrolling for the first time, use the company key “republic” when prompted.
- Call the Benefits Service Center at 888.850.1767, available Monday through Friday, 7 a.m. to 7 p.m. CT.
- Use the MyChoice benefits app.
Retirement Plans
At any time during the year, you have two ways to enroll in or make changes to your retirement plans:
- Visit Fidelity NetBenefits website. If you’re a first-time user, select Register as a new user to create your username and password.
- Speak to a live service representative by calling one of the dedicated Fidelity phone numbers below:
- 401(k) Savings Plan: 800.835.5095
- Employee Stock Purchase Plan (ESPP): 800.544.9354
When Can I Make Changes?
For the 401(k) Savings Plan and the ESPP, you don’t need to provide a reason to make changes. You can make changes at any time by logging in to your account at Fidelity NetBenefits website.
For most of your other benefits, outside of your new hire enrollment period and Open Enrollment, you can make changes to your benefits only if you experience a qualifying life event. Any changes to your benefits because of a qualifying life event must be made within 31 days of that event. It’s up to you to log in to the Benefits Service Center website or to call 888.850.1767 within that 31-day period.
Don’t forget, you’ll need to provide some documentation to back up your life event. Your documentation could include things like a marriage certificate, birth certificate or divorce decree. If you don’t get your life event documentation in on time, your next chance to make changes will be during the next Open Enrollment period.
